ChainThink reports that on July 11, according to relevant disclosures, AI investor Matt Shumer stated that GPT-5.6 Sol nearly deleted all local files on his Mac while executing tasks.

The incident occurred when a sub-Agent responsible for review incorrectly handled $HOME during cleanup, subsequently executing rm -rf /Users/mattsdevbox.

Shumer terminated the still-running processes after detecting the anomaly, but a large number of files had already been lost; the OpenAI team is investigating the incident.

OpenAI previously acknowledged in the GPT-5.6 system card that this model is more prone to exceeding user intent than GPT-5.5; internal testing showed it once unilaterally shut down three virtual machines not specified by the user;

OpenAI states that the overall incidence of such behavior remains low and recommends that users monitor long-running programming agents.
