Markets were concerned that rising commercial insurance costs would pressure profitability, but Goldman Sachs believes UnitedHealth's margin recovery exceeded expectations. UnitedHealth Group (UNH) reported adjusted EPS of $4.69 for Q3, significantly surpassing Goldman Sachs’ forecast of $3.90 and the Visible Alpha consensus of $4.06. The medical loss ratio came in at 88.4%, 160 basis points better than both Goldman Sachs’ and the consensus estimates.
Goldman Sachs maintained a Buy rating on UnitedHealth Group in its report issued on October 13, 2026, raising the target price from $490 to $506, representing a 33.9% upside from the closing price of $377.76 on October 13. Goldman Sachs believes that the third-quarter results validated its assessment of a dual-cycle margin recovery, with upside driven by UnitedHealthcare (UHC) and Optum Health. The adjusted EPS guidance for 2026 was raised to at least $20.50, with management indicating that 2027 consensus estimates may underestimate profit potential.
Medicare profit margin is nearing the target ceiling.
Medicare Advantage (MA) profit margins are now expected to approach the upper end of the 2% to 4% target range, up from prior expectations of around 3%. The pace of recovery has significantly outpaced expectations, less than a year after the implementation of pricing and benefit adjustments. UnitedHealth expects approximately 75% of members to be enrolled in plans rated 4 stars or higher by early 2028, up from the initial target of 69%.
UnitedHealth Group's third-quarter adjusted operating profit was $2.7 billion, exceeding Goldman Sachs' and consensus estimates by 44% and 26%, respectively. Adjusted operating margin improved to 3.1% from 2.1% in the same period last year. Management noted that utilization remains above historical levels but is stabilizing, and Medicare performance exceeded expectations.
Goldman Sachs believes that the recovery in MA profit margins is happening faster than expected, indicating that the incremental recovery potential is maturing more quickly. However, UnitedHealth's profit levers extend beyond this, with additional profit drivers in Optum Health, commercial insurance, and Medicaid.
Optum Health and Medicaid still have room.
Optum Health's adjusted operating margin for the third quarter was 2.6%, exceeding Goldman Sachs' expectation of 1.0% and the consensus estimate of 1.2%. Medical cost management, value-based care execution, and operational discipline continue to improve. Goldman Sachs believes the third-quarter beat is largely sustainable and will contribute to forward-looking earnings fundamentals.
Optum Health remains significantly below its long-term profit margin target of 6% to 8%. UnitedHealth has reaffirmed its goal of returning to this range by 2028. Profit growth in 2027 is supported by operational initiatives, contract optimization, and portfolio refinement.
Medicaid performance improved in Q3, driven by slowing trends in operational execution and utilization. Core Medicaid profit margin is expected to be slightly better than -1% in 2026, remaining the low point for margins. UnitedHealth aims to return to a 2% profit margin by 2028. Goldman Sachs believes rate adequacy is the key variable, and margin recovery primarily depends on improved state rate setting.
Commercial insurance cost trends exceeded expectations
Commercial health insurance cost trends exceeded initial expectations by 11%. Costs related to the Independent Dispute Resolution (IDR) process have accelerated, with arbitration rulings in some regions approaching 6% of total medical spending, representing billions of dollars in excess costs. IDR contributes approximately 100 basis points to trend pressure, with specialty pharmacy and provider coding contributing a similar magnitude.
UnitedHealth expects these high-cost trends to continue through 2027 and has priced its commercial insurance business accordingly. The recovery in commercial insurance margins is expected to extend beyond 2027. Goldman Sachs believes that commercial insurance cost trends are the primary near-term pressure on earnings.
ACA business profitability is expected to be approximately 0% in 2026, as UnitedHealth has decided to return profits to members. No such return is planned for 2027, and ACA profitability may improve to a low single-digit range, providing a modest tailwind for commercial insurance profitability.
The 2027 guidance suggests upside potential.
UnitedHealth raised its 2026 adjusted EPS guidance to at least $20.50, up from the previous range of $19.50 to $20.00. Goldman Sachs and the consensus estimate were $20.05 and $19.77, respectively. The midpoint represents approximately 25% growth.

Management indicated that the current 2027 consensus estimate (Visible Alpha: $22.54) may undervalue profit opportunities. The lower end of the 13% to 16% long-term growth framework is a reasonable starting point, based on the updated 2026 guidance, implying at least $23.17 in EPS for 2027.
Goldman Sachs raised its EPS estimates for 2026 to 2028 to $20.55, $23.50, and $26.75, respectively, and introduced a 2029 estimate of $30.35. Goldman Sachs believes there is upside to the 2027 consensus estimate, as UnitedHealth's earnings leverage extends beyond the recovery of MA profit margins.
Target price raised to $506
Goldman Sachs raised its price target from $490 to $506, based on a target P/E ratio of 19.5x (unchanged) applied to its 12-month forward earnings per share estimate. Goldman Sachs maintains a Buy rating.
Upside risks include a faster-than-expected recovery in MA profit margins, improved performance at Optum Health, and stronger commercial insurance pricing discipline. Downside risks include increased utilization, regulatory changes and scrutiny, and misalignment between pricing and utilization.
Goldman Sachs believes that UnitedHealth's margin recovery exceeds MA normalization. Additional profit levers from Optum Health, commercial insurance, and Medicaid drive growth beyond 2027. The faster-than-expected recovery in MA margins indicates that UnitedHealth's profit levers extend even further.
