Ethereum Prysm Client Experiences Mainnet Outage, Leading to Block and Attestation Loss

Ethereum’s mainnet faced a major outage on December 4 as the Prysm client struggled with resource exhaustion. The mainnet incident, during the Fusaka period, caused widespread beacon node failures, leading to 248 missing blocks across 42 epochs. Validators lost 382 ETH in rewards as participation dropped to 75%. The project team traced the issue to faulty attestation validation, which triggered repeated state replays. A temporary fix was deployed in v7.0, while v7.1 introduced a permanent solution. Network participation recovered to 95% by epoch 411480. The project team acknowledged communication gaps and testing limitations, pledging improvements.
