Most "agent failures" get blamed on the wrong thing! Someone spends a day rewording a prompt when the real problem was that nothing verified the output, or there was no stop condition, or the agent had zero memory of what it tried three steps ago. None of that lives at the prompt layer, so no amount of prompt tuning was ever going to fix it.
