The U.S. Occupational Safety and Health Administration (OSHA) has launched an investigation into the death of a worker at SpaceX’s Starbase launch site in southern Texas. The incident occurred in the early hours of May 15, and local law enforcement later confirmed that a worker died at the site; however, the circumstances and cause of death have not yet been disclosed.
Details of the incident have not been disclosed.
According to local media citing OSHA and local officials, the identity of the deceased has not been released, and the time of death is estimated to be approximately 4:17 a.m. local time on May 15. OSHA confirmed to the media that it has initiated an investigation into what appears to be a work-related incident.
Regulators stated that no further information will be disclosed until the investigation is complete. Based on previous procedures, such investigations may last several months.
Starbase's security record comes under renewed scrutiny
Starbase is the primary site for SpaceX’s Starship prototype testing and launches, as well as a large-scale construction site under continuous development. Due to the concurrent operations of testing, lifting, and construction, this location has raised safety concerns on multiple occasions over the past few years.
TechCrunch previously analyzed OSHA data, finding that Starbase had a significantly higher rate of workplace injuries compared to peers and was one of SpaceX’s riskiest operational sites. A 2023 Reuters investigation also revealed multiple previously unreported workplace injuries at SpaceX’s McGregor test site in Texas, including a fatal incident in 2014.
Has been issued multiple severe violation fines within the year.
In January, OSHA issued seven "serious" safety violations to SpaceX regarding a crane collapse incident at Starbase in June of last year, citing failures to inspect the crane as required.
Six of the penalties were imposed at the maximum amount, totaling $115,850. Federal records show that SpaceX is challenging these penalties.
This means that, prior to the latest fatal incident, Starbase’s safety management was already under continuous scrutiny by regulatory authorities. The findings of this investigation may further influence external perceptions of the site’s construction management and operational pace.
The incident occurred at a critical time.
The timing of this incident is sensitive. According to reports, SpaceX is preparing for the first launch of the upgraded Starship, while the market is also closely watching its potential capital market moves.
If the investigation takes a long time or new security issues are discovered, related developments may add additional pressure on launch scheduling, on-site construction management, and the company’s external communications.
